Frozen shoulder, medically known as adhesive capsulitis, is characterized by the loss of range of motion in a specific pattern, often referred to as a capsular pattern. The shoulder capsule becomes inflamed and tightens, restricting movement. This condition tends to follow a predictable course, affecting certain movements more than others.
The most telling sign of frozen shoulder is often the limitation in external rotation. To test this, start with your elbows bent at your sides and see how far you can rotate your arms outward. Normally, you should achieve about 70 degrees of external rotation. If you can't, it might indicate an issue.
If you can push your arm further using the other hand and it moves easily, you’re likely dealing with a rotator cuff issue, not frozen shoulder.

Both frozen shoulder and rotator cuff injuries can limit this motion, but the key is checking both active and passive range of motion. Use a stick to assist with passive movement; if the joint remains inflexible, it points more toward frozen shoulder.
The last test involves internal rotation, or the ability to reach behind your back. While not as commonly affected as the first two movements, a decrease in this range can still be an indicator. If you struggle to reach past your belt line, this may be a sign of developing frozen shoulder.
